CHC General Advice Line Service

Would you like the opportunity to speak to one of our nurses about the appropriate setting for your relative’s care? Perhaps you need advice on the difference between residential, nursing care and EMI care or perhaps you would like advice about finding a care home or how to get a Checklist assessment carried out for NHS Continuing Healthcare Funding. Maybe you need some advice about what  clinical services are available once someone has been discharged from hospital?

Perhaps you have a complicated procedural issue which you would like to discuss?

We offer this service in slots of one hour with a minimum of half an hour. Please note that the consideration of any documentation will be chargeable not just the call itself.
